Jesus
Hernandez
Executive Director
Background: Jesus has brought to Community Choice extensive
expertise and experience in start up and implementation of
programs, outreach and education experience with
disadvantaged
populations. Jesus completed a Bachelor degree from
Washington State University in Business Administration and a
Masters in Public Administration from University of
Washington.
He has diverse experience in administration of programs in workforce
development for people with disabilities and minorities, small
business development, youth leadership programs, strategic
planning, policy development, public education and workforce
investment. Jesus has served over ten years as an
elected School Board Member of the Wenatchee School District
and was appointed by both Governor Locke and Governor
Christine Gregoire to the state's Higher Education
Coordinating board. Jesus is actively involved in
various civic activities and non-profit boards in his
community.
Expertise: Grants administration and program implementation; outreach and
community development; program design; grant writing and review;
group facilitation; group training in cultural competency
in the workforce; health information systems implementation; marketing; bilingual, bi-literate, bicultural
in Spanish/English.
Maria
Ontiveros Administrative Assistant/Program Coordinator
As Administrative Assistant, Maria provides secretarial
support,
and coordination of board meetings and member communications.
She also puts together the bi-monthly Newsletter that all
Community Choice members receive. Maria has over seven years
of experience in the healthcare field and is bilingual in
Spanish/English. Maria is also the Coordinator of the
SHIBA HelpLine program serving Chelan, Douglas and Okanogan
counties. Our SHIBA HelpLine Volunteers provide assistance to clients
with eligibility, application and enrollment
in Medicare, Medicaid, Basic Health and other local, state and federal public and private health insurance
programs. They also provide outreach and public education to
help develop more informed healthcare consumers among Spanish
and English speaking communities.
Carmen
Alvarado

SHIBA HelpLine Program Coordinator, Moses Lake field office
As the SHIBA HelpLine Coordinator, Carmen works with our
SHIBA HelpLine volunteers to provide assistance to clients
with eligibility, application and enrollment
in Medicare, Medicaid, Basic Health and other local, state and federal public and private health insurance
programs. They also provide outreach and public education to
help develop more informed healthcare consumers among Spanish
and English speaking communities.
Janet
Hanke
Access Coordinator, Brewster field office
As Access Coordinator, Janet provides assistance clients
with eligibility, application and case management of enrollment
in local, state and federal public and private health insurance
programs. Janet also provides outreach and support to our
local community provider group serving the greater Brewster
and Omak area. Janet has over 15 years of experience working
with healthcare providers.
Nancy
Nash
Access Coordinator, Omak field office
As Access Coordinator, Nancy provides assistance to clients
with eligibility, application and case management of enrollment
in local, state and federal public and private health insurance
programs. Nancy also provides outreach and support to our
local community provider group serving the Omak/Okanogan
community and vicinity. Nancy is bilingual/bi-literate/bicultural in
Spanish/English and provides cultural awareness programs in
her community.
Amy Lyons
Bookkeeper
Amy provides bookkeeping, accounts payable /receivable and
billing for Community Choice. May has significant
experience in bookkeeping and provides her service to numerous
non-profit organizations in the Wenatchee Valley.
