Community Choice Healthcare Network physician members are committed to providing excellent health
care to their patients. Our ability to improve patient services is greatly
enhanced by the feedback that our patients provide us. To improve our services,
please take a few minutes to fill out a medical service evaluation.
To maintain the integrity of the data as well as patient privacy and anonymity each patient is provided an evaluation number
during a doctor's visit. Patients use this number to complete
their evaluation. Community Choice will maintain your
anonymity and provide your physician with your valuable input.
